The only 'catch' to our 4xY program is that it's not yet available in communities we are not a Health Partner with.
For those communities that we do Partner with, we require a minimum number of residents to participate in the 4xY program. That minimum is specific to the individual community.
The 4xY program requires a minimum commitment of 12 consecutive months. In the event this commitment is not met, all fees for services rendered revert back to their regular fees per procedure performed. Please call us for complete details.
